When a student has made multiple attempts and they appear contiguously in the table, depending on sorting, the student's name is only displayed on the first line of these contiguous attempts. This corresponds to cases no 1 and no 2 of your example.
When the attempts do not appear contiguously, the student's name is displayed for each attempt. This corresponds to case no 3 of your example.
This is not a bug, but rather the expected behaviour.
