Seems peculiar to me that the "+ Add an activity" is on the far left, under the Hidden from students text. I don't see that happening to me in a clean 3.5 site with Boost. It aligns normally to the right. Do you have customizations in the theme at all? in HTML or css?
Another way to troubleshoot this is to create a new role from the Manager archetype, without any changes. Then assign that to a test user and have them go into a course that does not have the auto enrolment plugin, and test. That would verify if the issue is with the role & permissions or not.
