Installing and upgrading help

I've split this from the original discussion - please don't "hijack" original threads.

We do need some basic information to go on.

- When do you see this (some pages, every page)?
- What Moodle version?
- What PHP version?
- Switch on Debugging - you should get additional debugging info which will help
