Adding Notes Column/Upload Documentation

I manage two Moodle platforms (version 3.8.1+ and 3.9.1+). These platforms are being used to run wilderness medicine courses and other types of trainings. We have a need for adding a notes column to the gradebook not visible by students. This column would need access to the text editor so we can upload documents like Failure/Downgrade reports and paperwork that is signed in class by the student. Not all students have access to internet and some of the locations we teach also do not have internet. Does anyone know if this is a possibility?

I am aware of the Notes feature in the student profile. However, the textbox does not include the text editor and thus, files cannot be uploaded.

Add an assignment activity which is only used to upload your documentation.
The activity will become a Gradebook item, that you could hide from the students view.
