Stopping teachers from accidentally regrading reopened attempts that don't have a new submission AND/OR reverting back once this is done

Hi,

We have set up and assignment that a student can choose to do up to 3 times if they would like to aim for a better grade.

Once the first attempt is graded, the teacher reopens the attempt and the student can happily resubmit.

The problem we are having is that teachers for some reason end up re-grading the reopened attempt even though the student hasn't submitted anything ( I suspect they think the first attempt didn't register, or they think they didn't do it and do it again). 

When this happens, the student looses the next attempt without using it.

The immediate solution is to re-enforce to teachers not to grade "reopened attempts".

My questions:

1) is there a way for the system to keep you from grading reopened attempts (as in the status being "reopened" and not "submitted for grading"?

2) once this is done, is there a way to roll-back and reset the second attempt (erase the grade, erase the feedback) from that front-end interface.
