Cloze question - Potential for cheating?

Hello, 

So the question I have been asked was about potential for cheating/gaming the Cloze question.

Basically the student needs to fill in the symptoms however it was pointed out that they could put the same answer in each box and get the full marks, 

Is there anyway to also have it that if they have selected one "symptom"  then that one can't be used again or not marked as a full point?
