I used your suggestion to null out the alternate URL and it worked. I can login via the normal Moodle manual login screen, so it seems like our dual login page is the source of the problem. Somehow it is breaking the exchange of password and username for both the manual and Shibboleth logins.
This is the same dual login page that we've used for years on our Production system ( with the URL references changed for the new system) and is currently working on my test system. I'm contacting the author of that page to see if he has any ideas.
