Did you copy and paste text and images from a Word file onto the page? If so, that would cause this behaviour. The Word import plugin for Books (https://moodle.org/plugins/booktool_wordimport) would resolve this. Alternatively, if you linked to external image URLs that you no longer have access to would give the same problem, I think.
