Well, "for a teacher to evaluate their course" is really something that happens inside the teacher's head. Any use of technology is secondary.
But, if you wanted to guide their refection, then you could make something to help, which could be as simple as one page of text, with a few bullet points they might like to consider. (What those points should be is a very interesting question.)
Then, if it seems that more technology might contribute something, then some sort of questionnaire or survey might be worth the effort.
