devices should be registered when a user adds a site on the device, or if the user opens the app and he has a site already added.
Regarding the push notifications, I'm not an expert in Airnotifier so I cannot help you with that. I think Airnotifier has a log, I'd recommend you to check if Moodle notifications reach Airnotifier or not, that way you can tell if it's a problem between Moodle-Airnotifier communication or the problem is between Airnotifier and the device.
