Has anyone customised the offline marking sheet in terms of the columns that can be included? I'm not sure if this is possible without modifying code and I can't find mention of this elsewhere (although I've only searched briefly).

I think I've answered this myself. The columns relating to names are controlled by the global setting "Show user identity" (showuseridentity) under Users > User policies. Of course this has wider implications on other tables around the site, so it's not ideal.
