Hello. I've just had this problem when I bulk uploaded my courses. The excel CSV file was saved as CSV (comma delimited) and when i uploaded it in my moodle website i chose CSV-UTF 8.
What i had to do is to change my excel file to CSV-UTF 8 (it has to be the same as the file you chose to upload in moodle)
So, try to save your CSV file as CSV UTF-8 (comma delimited)
