Cannot duplicate activities in Moodle 3.5.5

We are on Moodle 3.5.5 (Build: 20190311) and currently using the Adaptable theme.

I have an instructor, who was duplicating multiple quizzes, each with random questions, in one of her courses yesterday, who can not duplicate anything in any of her courses today.  She is the only user with this issue at this time.

but both bug trackers indicate that problem was fixed in Moodle versions 3.5.4, 3.6.2:

We are planning on moving from the Adaptable theme to one that supports Bootstrap 4 in the summer, but this instructor needs a solution before then ... and it doesn't sound like it is a theme-based issue from the discussions I have read.  If someone can provide a solution, it would be appreciated.

Hello Susan,

Are the questions placed in the system context?

Are there plugins that could create this problem?

Questions are saved at the course level. 

We haven't changed any plugins, since last summer and none that relate to the quiz activity.

I am trying to replicate what happened with debugging on, but so far not happening.  I suspect it is because my test instructor doesn't duplicate quizzes on a regular basis, while the instructor who reported the problems does.

Hello Susan,

I guess that you also tried to backup and restore the quizzes or the entire course.

Someone more knowledgeable should probably help you.

I have almost eight years administering Moodle, but don't have the PHP/SQL, etc. necessary to start changing background coding; my co-worker gets to deal with that end of things smile.

It is actually a bigger problem than not being able to duplicate quizzes in one course.  I restored the course that seems to be where the problem started and added our test instructor to it.  The test instructor can duplicate the quizzes and everything else in the course.  The instructor who reported the problem, can 't duplicate anything in the original course or any of her other courses.  Based on this, I am thinking that the problem is most likely tied to the user rather than the activity or course.

Worse case scenario, we can create a new account for the instructor, but we would like to know how to prevent this happening in the future and how to fix it, if possible, without creating new accounts.
