the Feedback activity is the most suitable for the task you describe. You can download the data as Excel and you also have some results display on the activity itself. Regarding the recurrence, I'd save an "empty" activity as a template and duplicate it as many times as needed. Ideally, users should be informed by email that a new questionnaire is available. I think this can be achieved with the event monitoring rules, but I've never tried it with a feedback. Or you can inform them manually each time faculty members have to submit a new questionnaire.
