We are encountering a problem with how the questionnaire results display:

Another example - only one response, so the error in calculations is very obvious:
So far we have identified 3 out of 13 questionnaires that are displaying this way. All 13 questionnaires were created from the same backup file with only the instructor and course names being changed.
