The PHP version is 7.1 and Mysql 5.7.
When you say "manual installation". Do you mean installing the plugin from brand new?
We are stuck and can't upgrade to 3.7.x either. The upgrade just keeps running until Moodle times out with an error:
Fatal error: Maximum execution time of 600 seconds exceeded in \htdocs\moodle\lib\dml\mysqli_native_moodle_database.php on line 730
I haven't tried 3.7.1 yet because it was crashing our weekly Moodle update.
We have 185 records in mdl_questionnaire and 1,492 records in mdl_questionnaire_question.
3.6.1 is running fine. Is there any way we can run the update via command line?
We are running MariaDB 10.3.20.
We have 73625 entries in the mdl_questionnaire_question and 959525 entries in the mdl_questionnaire_response_rank table.
Innotop shows many queries per second, but a repetition of "Removing tmp tables", "Opening tables", etc. I suspect an inefficient way of updating the tables in the function move_nameddegree_choices in mod/questionnaire/classes/question/rate.php

(update) the upgrade eventually terminated, it took 11552 seconds (3.2 hours).
I’m having the same error. After trying several times and cancel the update I notice that something is missing.
The Questionnaire is missing on the Administration block, between Page and Quiz:

I tried again with your fix but same thing happened.
5 minutes after starting the upgrade it stopped:
Then, the only option was to cancel:
Nevertheless it is functional with 3.6.1 version

I upgraded to the latest release 3.7.2 with success.
