You're not doing anything wrong. Currently, there is no feedback that pushes to the user report from a Quiz. There are a few discussions you can follow regarding this, such as: https://moodle.org/mod/forum/discuss.php?d=120360

John, I'm having same issue. What is the point of the Feedback column if nothing ever populates it? Are there any ways to make the feedback comments show up in this column?

Teachers always have the option to manually add feedback in the grader report, this unfortunately locks the grade though for students who have more attempts.

Thanks for getting back to me. My problem is that I have utterly no idea how to get anything at all into the Feedback column in the user report. Perhaps the problem is that I just don't understand Moodle.
You say that Teachers always have the option to manually add feedback, but I can't figure out how that is done. When we grade and add feedback it never shows up in that column.
