Moodle 3.3 Failed Connect to LDAP

I have moodle version 3.3 and i'd like to integrate to my windows LDAP AD server. I have configured it in Plugin > Authentication but the test settings options is greyed out.

When i tried to login using ldap authentication, the error said 

LDAP-module cannot connect to any servers: Server: '172.16.104.251', Connection: 'Resource id #82', Bind result: ''

My starting point with LDAP issues is to eliminate Moodle. Find/get an LDAP search tool (I don't use Windows so can't recommend one for that platform) and run it on the Moodle server. Can that connect to your AD instance?

Most LDAP connection problems are permissions or firewall related.
