Going to /login/logout.php on Moodle.org as well as on several other sites that I manage doesn’t redirect me to username/index.php - it just stays on login/logout.php and asks me if I really want to log out.
Are there any server level redirects set up? In an htaccess file for example?
What theme are you using? Can you replicate this in a standard Moodle theme like Boost?
When you say the problem just started... can you narrow that down? When? And did it coincide with any Moodle upgrade, plugins being install/deleted/upgraded, server OS upgrades, etc?
