Turns out - it's to do with the Safari/iPad default video player misinterpreting the 270 degree orientation metadata as 'needing to be a mirror image'. Not an issue with Moodle at all.
The only solution I have found is to reset the orientation metadata to 0 degrees, however that breaks the video orientation for EVERY OTHER DEVICE.
