I'm using the Generico filter and atto plugin and it works great.
is there a way to make a template in which the dialog will contain a select box with pre defined options with their values  from the user ?

or a radio button like so:

right now it seems only possible for the dialog to contain text input.

I'm using moodle 3.5.1

I'm sure there would be, but there are three problems with this. One is development, you will have to develop the template yourself, then test it thoroughly to make sure it plays nicely within the environment it is being used in. The second is that as you update your Moodle, you really need to update your template to make sure it still works as it should. The last is this is a huge time commitment, and if you move on to another job, will there be someone to take over from you?
You may want to start with the Moodle development documentation to gain insight as to how you can do what you want to do.
I don't want to turn you off doing this, just want you to make sure you know what you are letting yourself in for, it's too easy to be monstered by what initially seems to be a small adaptation. Good luck,.
