Thanks for going down the rabbit hole!
We've already been quite a long way down the same rabbit hole - in fact we worked out the sql contained in the remove_unused_questions task for ourselves, and have been executing it directly through cron/mysql since before the Moodle task fix was published. This dealt with the most extreme 'duplicate question' problem, which was resulting in literally millions of records in the question table, most useless orphans.
However, our outstanding issue is that even once we clear out the 'orphan' questions (ie not linked to a quiz slot), we are still getting mysterious duplicate questions appearing within the same question bank.
You have definitely helped us though - I now know that duplicate stamp/version is acceptable when in different contexts
Our plan now is to write an admin page to identify all duplicates within a context, this will both to identify them so they can be deleted, and also alert us to new ones much sooner, so we might be able to track down their source
