I'm searching for plugin which will allow user to fill form and show their interest in course . 

Its like showing interest in course form if course enrollment date is passed or course is over.

Is there such plugin available .

Sorry to answer you so late, I recommend using the Questionnaire Module. If it is not on your Moodle, ask your admin to add it.
