We are facing some issues with this plugin.
First of all we are getting this message when we try to add questionnaire to any class:
After that we decided to update this plugin from version 3.5.0 (Build - 2018061900) to the newest one Release 3.5.4 (Build - 2019032100)
And then we click "Update" we are getting this error:
How can we solve this?
Moodle version - 3.6.2+

Somehow, you have gotten your versions out if sync with your database.
The table referred to in the error, "questionnaire_attempts", was removed in version 2018050102, and released in 3.5.1. If you upgraded from 3.5.0, the table would be removed during that upgrade. The error is indicating that it can't find the table, meaning it was already removed before you started the upgrade.
Did you have a partial upgrade fail at some point? Did you manually change your database?

Maybe we had some fails during upgrade from 3.5.0 version i don't remember.
We solved this problem by completely deleting this plugin and installing the newest version (3.5.4)
