I'm on Moodle 3.0 and also having this problem.
I've set the criteria, ensured badge issuing is enabled, course completion is enabled, activities are set up to auto-complete when activity is completed, but the badge simply is not issued to the learner.
I've used badges before as a test 2 years ago and it worked then. Has there been an update recently that's causing this?
