Another error is that once a student responds and clicks to see their own responses, it displays the top answer when questions are based on a drop-down menu. In this example, my sample student selected the middle option in each of the drop-down lists, yet when displaying Your Response, the display defaults back to the top answer in each list:
What the teacher sees appears to be correct.
There were also reports of a 1969 completion date, but I haven't been able to replicate that.

I do not understand your problem. Your screenshot shows a questionnaire being answered, it does not show a questionnaire report. The "Your response" report should show the actual student's responses to the right of the drop-down lists, as per my attached screenshot.
Which versions of Questionnaire and Moodle are you using?
Not forgotten, just the start of our semester and limited staff available. The problem is that when students complete the questionnaire and review their responses, the questionnaire defaults back to the original list of options - this video hopefully explains what I am referring to
What the instructor sees is the accumulated responses for the class so she gets a response she can use and is fine.
The version of the questionnaire we are using is 2018050101 and here are the settings for the questionnaire (everything else has been left at the basic default)

Indeed your video shows that the display for the student viewing their own response is not what it should be. There are actually 2 (related?) problems with the display you are getting.
You could try to upgrade to a more recent version of Questionnaire.
Another possibility is the Moodle visual theme you are using (what is it?). You could try switching to a standard theme such as Boost.
Finally, which platform and which browser are you using? You could try switching to different ones to check that the problem is still there.
I don't think it's related to your questionnaire settings, anyway.

The Questionnaire plugin has been updated to the most recent version (and created database errors in a few courses recycling very old questionnaires), but it didn't resolve the display problem. This leaves Theme as the major suspect and we have requested that the Essential and Adaptable themes be added to our test server, so we can check it out there. If that resolves the problem, I will post an update.
