Is it possible to add a Cohort to the Course Category level and have it propagate into the courses in the category?
Do I have to add the cohort sync to each course under the category?

When you create a cohort in the course category level, it will be visible only into the courses in this category but you will need to add the cohort sync to each course.

Please note, there are also multiple plugins available that extend the cohorts feature. You might find https://moodle.org/plugins/local_cohortrole useful, as just an example.
