Try to check it from changing to Boost theme.
Same problem here, it doesn't show in Dashboard or any course pages, only in the /admin/ pages using theme_lambda, using theme_boost it shows fine, using theme_clean it doesn't work as well. First I thought it was theme_lambda issue but once it's not showing also in theme_clean, I'm not sure anymore.
I could fix the issue after I found an instance of the block in this page /course/index.php, and change the settings of the block to show in "Any Page".
