This is surely the sign that something is wrong in your XML file. I admit the error message could be more explicit, but it is not easy to tell the user what is wrong in his file.
Was your xml file generated by export from question bank, generated by some tool, or manually edited ?
To tell you what is the problem in your file, we would need (if this is possible) to look at its content.
