Hi,

We recently upgraded to 3.5.1+ and it seems to have broken the Question Bank. I'm wondering if anyone has ideas on how to troubleshoot this. The Question Bank itself seems to work fine, and we're able to add/edit/remove questions normally, but it hangs when you try to add a question to a quiz.

This is happening in every course, across two different deployments that we upgraded at the same time.

I've attached screenshots to help illustrate. Everything looks normal in the Question Bank, but when you try to add a question to the quiz everything is greyed out and the loading animation keeps spinning forever.
