In my moodle installation I used to get add an activity or resource popup like the one below. But all of a sudden it has now changed to 2 select menus - activity and resource selection. It is fully functional, but lacks the expected look and feel.
I am not able to figure out what I have misconfigured. Pls help

Well, I have a v3.4 on a Ngnix NAS, with the More theme. It all works so has the Theme been changed recently? If not, then it might be faster just to clear your Moodle code and replace it with the most recent stable version. You can go looking for the problem, but if it is not a theme related change, then why waste time on it?
Isn't that the fall back display when there is a JavaScript issue? Theme or browser looks like a good places to start.
