I would need to be able to create an additional form template in the database activity, is this possible and if so (as I hope) how to achieve this ... A second solution would be for me to be able to link two database activities (using the same data).

Both of your questions are not easy.
Moodle database activity only has one template for List View and one for Single View. You can try:
