The JS error was caused by some additional code I put in and has now been fixed. Just to double check I also commented out all of my custom code, but unfortunately I am still seeing a black screen when I try to add a topic.

I had the same problem but I solved it changing Moodle Theme (from bootstrap to clean)

Unfortunately we have done a fair bit of customizing to our theme making it difficult to switch, but thanks for the reply.

Unfortunately we haven't solved it yet and it has been put on the backburner.
My theme is on the clean one and we still have the issue.
I have version 3.4
