We have already open a bug tracker, but moderator ask me to submit here the problem to find someone who have any ideas.
When we try to upload a file into an assigment we receive this error:

Now the situation become more complicated, because if the user press the continue button and return again into the assigment the file was uploaded correctly but the status was "not submitted" otherwise if the user do a page refresh (F5) without leave the error page the submission of assigment gone right and the page are load normaly.
The problem occur only for the first submisison if we upload a second file the error doesn't occur.
We use Chrome browser and the php error log doesn't show any information.
We have already checked the file permisison and cleared the cache but the problem persist.
