I think it's possible with "astuce" wink until a real solution

    create an assignment (or database) in your course
    put this activity in orphaned section (until 3.2)
    copy the link to this activity
    paste this link into the survey

Yes, student has 2 activity, he has to send questionnaire after upload assignment, but it's one solution I think...

You can associate a achievment criteria to be sure student do all actions...
