Cannot add entries, no error message

I have a database activity that looks fine but entries cannot be added for some reason. The templates are all set up. When I try to Add Entry, nothing happens, no error, etc. There are only 4 fields (URL, text, upload file, text), all optional. It says there are no entries in the DB, exporting confirms this, but it also says that 37 people have completed the activity (completion requirement = 1 entry). 

Ideas? Using Moodle 3.1. I have other DBs that work fine.

Hi Margaret,

You have 9 fields, but you only use 3 in your 'Add entry' form. There must be a 'required field' in one of the other 6 and they never got a chance to be filled.

Remove the unused fields, then everything should be ok.
