Uploade form not loading after click on 'media' button in Tinymce

Moodle 2.8.3

Theme bootstrapbase: 2.3.0

In Tinymce editor,

to insert a file, clicked on 'media' button-> then clicked on find or upload a  sound ... -> then File picker window  opened.

Clicked in left menu( upload  a file button ),  but no 'Choose file' button, nothing is showing, its total empty.

Anybody has any idea, what is the reason behind it?

FYI

There is no error in the console ( please see the attachment ) . How can I debug or find out the problem.
