How did you upgrade (or what instructions did you follow)?
Switch on Debugging (all the way up to Developer and display errors). Try again. Do you see extra messages? Also check your web server's error log for relevant errors (don't ignore this bit).

Well...
We did all written about the upgrade process:
1 - Backup the site/Database
2 - Move all content to a copy location
3- Upload the new version of Moodle
4- copy the config file from the moved to the actual site.
5 - Start the install/upgrade process...
We did it about a month ago.. but until now we noticed the errors...
I will try the debug mode... and see what it says...
I will handle to you, maybe you can give an answer...

The issues about the unseen delete confirmation page/warnings are related to the adaptable theme...
The only problem we do not know about is on pending activities error..
