Thank you for your quick response! I will absolutely update our version of Moodle as soon as possible.

In the meantime, I checked all settings on our courses and we have set each to 'force new attempt = yes'. I have attached a screenshot of the settings I use on all courses. Is there perhaps something else here that should be changed, to rectify the problem?
