I have Moodle version 3.1.1 and in every theme (including clean) long titles are not wrapping properly and they are overlapping with other elements. Field Custom css is empty.
Probably bad CSS in a plugin.

Any idea how to find it? I have a lot of plugins installed, I have to remove one by one to find which one has bad css?

Did you try to find the origin of the CSS using Firebug?
I agree with Gareth that the problem could be a wrong CSS from a plugin. But you can try to find which one using Firebug with Designer mode enabled instead removing plugins.

Problem is solved! We have upgrade php from 5.4 to 5.6, and now everything looks fine.
