this is a relatively large module for a client (approx 360 slides) so i will need to see if i can cut it down to just the section causing the issue.
We produced the file in scorm 1.2 format for our client.
the client has added it on to their moodle LMS. we have no access to this, so i am unsure how they have added it. i can contact the client to find out.

sorry for the very late response (we have limited time for support), we did some tests and it seems that the problem is in the SCORM package.
The package in the browser uses flash, meanwhile in the app uses an HTML5 version. The HTML5 version is the one not working (I tested it in a browser disabling flash, in chrome via chrome://plugins/ and it doesn't work either).
So I'd recommend you to contact with your tools' support and report the problem, in the reproduction steps you must indicated that this happens only in the HTML5 version
