Yeah, I am afraid it can happen. IIRC, the submission form has only the title set as required. So technically it is valid to submit work consisting of just the title. In case of your student, maybe she had the text in the text field selected and pressed delete by accident or so. I believe it was a rarely happening human mistake, rather than technical issue in Moodle.
