On the image you have given - click on the 'User Profile Fields' link in the Admin menu
That will allow you to create/edit fields defined by the admin.
If you want to edit the core fields that already exist, then that would take some hacking in the code - but you would need to be very careful that other parts of Moodle do not break because of a missing value from the profile then.
