Hope you can help here. Our users have started getting other people's Moodle completion notifications like below.

Student A has completed
'Assignment 1-8: Applied Mapping Skills'
in course 'Geography'
You can review this attempt at
User A is not the teacher of course 'Geography' it is owned by User B
---------------------------------------------------------------------------------------------------------
Student A is enrolled in the course 'Geography' class who teacher is User B, but his assignment notifications are going to User A.
This has happened in the past but I can't remember what I did to fix it.
