Hello Al, thank for sending me the zip file and trying on Moodle 2.8 as well.
I have tested your zip file on the Moodle 2.8 installation and had the same issue.
Just to be sure I tested both ZIP files on a Moodle 2.9 installation, which caused no issues.
So my best guess it's either a setting on the server or in Moodle itself, which I have to be looking for.
