I too am looking for a solution to this. I thought it would be as straight forward as backing up and restoring/importing but that does not seem to be the case.
I have a complex gradebook of only manually added grade items and categories and would just like to be able to create new courses with the same items.
Is there anyway currently of doing this?

Do you have admin capabilities on your site? Sometimes we can back up more than an instructor can. I have backed up my course with categories, grade items, etc. and restored into another course with activities already in it, merging with the current course. In the back up, the only thing I clicked off was the users enrolled in the course so anything that goes with that was turned off as well. The gradebook categories, and all activities came over. Anything that was in the course already before the restore into it, was positioned outside the categories. I was working in 2.9
I do have admin permissions on the site and have actually solved the issue. After trying a few backup/restores of the course I noticed that a particular activity from an installed plugin (auto_attendance) was also not being copied over for some reason.
On a hunch I decided to remove this activity from the course before backing up thinking it might be causing some type of conflict since it does post to the grade book and voila, all my items and categories restored.
Guess there is some bug in the plugin that causes the gradebook to get corrupted.

Does anyone know how to do this with ONLY the gradebook structure? I don't want to move any items just create a 'gradebook structure template' and then use that template to create the gradebook structures in other courses. Any ideas?
When you say gradebook structure, you mean the Gradebook Categories?
If so, i'm also looking for a way to do this.
In the meantime, I have a template course that has my gradebook categories all set up, so I just clone the template course.
I guess that's the way to do it...set up a template course with only the gradebook done...and then import the course. You are saying this works?
Keith - Yes, whether you backup and restore, or go to a new course and import, it does include the gradebook categories setup.
i don't know what back up setting controls this though. These are what I use:
(My guess is that it's not controlled by any of these but by the simple fact that you are importing/restoring at the course level versus importing at the activity level). I say this because did you ever notice how course groupings/groups also get transferred from once course to another even if you just import one activity (at the course level)?
