Plugin that allows user to fill in and submit a form in a course

Hi all,

I'm looking for a plugin that will allow me to add a resource or activity to a course.

This plugin will allow a user to enter information into some fields, like a sort of contact form, and send that to the sites email.

How about the standard Feedback activity? Present on all Moodle sites but disabled by default.

Looks great, as the users can enter data into fields. Thanks.

Though I'm not quite sure at the moment how and where the emails with data will be sent.

Alex - regarding where the feedback results are sent, they are sent to the teacher in the course who added the instance of the Feedback. See the documentation

The Database tool would also work well for this (probably)
