SCORM issues like this are really hard to trace. They can be related to the specific environment (browser type/version etc) or the package itself. If you can reproduce the issue reliably while running the SCORM debugger it can often provide more useful output - but often it is really hard to find the correct scenario that allows you to reliably reproduce it.
