I kindly request help for this as i am searching for a solution for some time now...
Images (thumbnails too) are not showing and i think the themes are not showing ok too. During the installation, the enviroment check page was always showing something about slasharguments. Now i have read the documentation. I have "Acceptpathinfo on" on apache config. No matter what i did the check page showed the slasharguments error. Because i have an older PHP in the server, therefore i used mod_fcgid to use PHP 5.9. Can mod_fcgi provoke the slasharguments error and images not showing correctly? Intersting, the enviroment page after i install moodle, doesn't show any errors about slashes anymore.
I have uploaded a image with no space or funky characters to a profile photo, but still no image.
Moodle 2.9 (with pt_br as the only language)
Apache 2.2 (with "Acceptpathinfo on")
PHP 5.6.9 (with mod_fcgid) (cgi.fix_pathinfo is set to 1)
GD extension is activated
PostgreSQL 9.4 (moodle DB is UTF8)
I have activated the debug options on php.ini and Moodle's config.php
Moodledata is 777
I have made these tests and all passed except the last one. (from discussion and post 278916#p1199031)
phpinfo.php/filename with a whitespace
Am i missing something obvious here?

EDIT: i have changed the theme Clean to the Legacy, the logo and theme images appear now! Maybe the clean theme looked a bit too basic that i tought something was missing! haha
But still, profile images are not showing
