I'm experiencing a similar issue when previewing test items, but seems to be restricted to calculated item types. There doesn't seem to be any clear indication of a cause, but some searching has indicated possible problems with caching setups. I'll let you know if I come up with any viable solutions. Hopefully some others may have some ideas here.

I'm having the same issue in two different platforms. It looks like a moodle bug.
The platforms are in debug mode but they are not showing any specific error.
Moodle version: Moodle 2.9+ (Build: 20150521)
Theme: Essentials
PHP Version 5.4.19
¿Maybe an limit PHP memory issue?
The rest of the platform is working properly, it's just with Tests.
